Position Summary: The Center for Development and Disability (CDD), established in 1990, is New Mexico's University Center for Excellence in Developmental Disabilities Education, Research and Service. These centers, authorized by the Developmental Disabilities Assistance and Bill of Rights Act, build the capacities of states and communities to respond to the needs of individuals with developmental disabilities and their families.



CDD's Medically Fragile Case Management Program (MFCMP) is seeking a RN/Case Manager to provide comprehensive case management services for individuals who are medically fragile and their families. The Case Manager assists the families to access community based health care services, home visits, health care assessments, and assists in transition planning to community from the hospital as needed. The Case Manager works with the individual, family and the interdisciplinary health care team to develop a comprehensive health care plan to assist in the evaluation, implementation and coordination of services through the health care team and community based providers.



Having two or more years in pediatric and/or community health nursing is preferred; also experience working with individuals who are medically fragile as well as having developmental disabilities. The case manager must also have the ability to work autonomously, be a team leader as well as team member.



RN/case management services are provided statewide. Travel is required in southern New Mexico and may be required to different regions of New Mexico. The Case Manager must travel to rural and frontier areas as needed.

Minimum Qualifications: Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.

Bachelor's degree in Nursing or a related clinical field; at least 1 year of work experience that can be demonstrated to be applicable to the duties listed in the job description.

State of New Mexico Licensed Registered Nurse or licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the "multistate privilege to practice" compact with New Mexico.



Preferred Qualifications:

  • 2 plus years experience with Pediatric and/or community health nursing
  • Home visit experience
  • Experience working with families and individuals who are medically fragile
  • Experience working with the interdisciplinary team as a team leader and team member
  • Experience working with community providers in southern New Mexico
  • Experience accessing resources and networking
  • Supervisory experience with other nurses and/or case managers in terms of training, daily work and prioritization of duties
  • Bilingual English/Spanish
